Please arrive 15 minutes prior to your scheduled appointment.

Your appointment may be rescheduled if you are more than 15 minutes late.

We understand that life happens. If you need to cancel your appointment, please endeavor to cancel at least 24 hours in advance. You may be subject to a fee if you cancel less than 24 hours in advance.

If you have a true emergency, please call 911 and go to the nearest ER.

Please see the insurances accepted under the “About Us” tab. If your insurance is not listed, you can call the clinic at 682 882 6060 to verify

If you have not been seen in the clinic for 6 months or more, please call the clinic to schedule an appointment for any medication refills, except if the refills have already been approved by your provider.

We will only fill out paperwork for our already established patients. Please allow at least 1 week for completion of paperwork.

Please fill out the new patient forms under “New Patient” on our website prior to your scheduled appointment. If you are unable to do so, please arrive about 25 minutes prior to your appointment to allow time to fill out the forms.
